Introduction: Self-Administering Zarzio at Home

Many patients prescribed Zarzio (filgrastim) will administer their injections at home rather than visiting a clinic for every dose. While this can feel daunting at first, most people become comfortable with the process quickly. This guide walks through the key steps and considerations — but it does not replace the training and instructions provided by your healthcare team. Always follow their specific guidance first.

Before You Begin: Preparation Checklist

  • Wash your hands thoroughly with soap and water for at least 20 seconds
  • Remove the Zarzio syringe from the refrigerator and allow it to reach room temperature for approximately 30 minutes — injecting cold medication can be more uncomfortable
  • Check the expiry date on the pre-filled syringe
  • Inspect the liquid — it should be clear and colourless; do not use if it appears cloudy or contains particles
  • Prepare a clean, flat surface and gather your sharps disposal container

Choosing an Injection Site

Zarzio is most commonly given as a subcutaneous injection — into the fatty tissue just beneath the skin. Suitable sites include:

  • The abdomen (stomach area), at least 5 cm from the navel
  • The front of the thigh
  • The outer upper arm (usually given by a caregiver)

Rotate injection sites with each dose to avoid skin irritation and the build-up of hard tissue (lipohypertrophy) at any single location. Keep a simple record if it helps you track which site you used last.

Step-by-Step Injection Technique

  1. Clean the chosen skin area with an alcohol swab and allow it to air dry completely
  2. Remove the needle cap carefully, without touching the needle
  3. Gently pinch a fold of skin at the injection site with one hand
  4. Hold the syringe like a pencil and insert the needle at a 45–90 degree angle, depending on the amount of fatty tissue present
  5. Push the plunger slowly and steadily until the syringe is empty
  6. Withdraw the needle smoothly at the same angle it entered
  7. Apply gentle pressure with a dry cotton ball or gauze — do not rub the site
  8. Dispose of the used syringe immediately into your sharps container

Storing Zarzio Correctly

ConditionGuidance
Normal storageRefrigerate at 2°C–8°C; do not freeze
Out of refrigeratorMay be kept at room temperature (up to 30°C) for a single period of up to 7 days
Light exposureKeep in original packaging to protect from light
Frozen accidentallyDo not use; consult your pharmacist

Missed Doses

If you miss a scheduled dose, contact your oncology nurse or prescriber for advice. Do not take a double dose to compensate. Your treatment schedule is carefully designed around your chemotherapy cycle, so any adjustments should be made by your clinical team.

When to Seek Medical Advice Immediately

Contact your healthcare provider or seek urgent medical attention if you experience:

  • Signs of a serious allergic reaction (difficulty breathing, swelling of the face or throat, rapid heartbeat)
  • Sudden or severe pain in the upper left abdomen or shoulder tip (possible spleen-related symptom)
  • Chest pain or shortness of breath
  • Signs of infection at the injection site (redness, warmth, pus)
